Assessment-Findings-Plan (6 steps): this is where most people start, unless you know yourself quite well and already know how to connect the dots to create a healthy and purposeful life. Founder's price: $350

6 STEPS:

1: PRE-QUESTIONNAIRE

Once we set-up your appointment, I will send you a pre-questionnaire. 20 questions or so--as a way for me to get to know you in a meaningful way. You will complete and submit before we meet in person. I review, to prepare for step two.

2: ASSESSMENT

In-person or virtually. Occupational profile and additional assessments as indicated from the pre-questionnaire results. I review results--in combination with pre-questionnaire– to contribute to findings.

3: FINDINGS

In-person or virtually. We meet to talk about what was discovered. What stands out? What areas in life are working or not working? It you want to make adjustments, what would be next steps? Which leads to step four.

4: PLAN

In-person or virtually. RECOMMENDATIONS, RESOURCES, & REFERRALS This is where we develop a plan to help you get what you want in your life--toward health and purpose. If specific referrals are indicated such as trauma-based counseling, EMDR (spell out), physical therapist, psychiatrist, primary care physician, etc. I will assist. I provide specific resources to support your plan for next steps.

5: PLAN UPDATE and ACCOUNTABILITY

In-person or virtually. We meet approximately 1-3 weeks after our last meeting. A re-assessment of sorts, to see how you're feeling, thinking, and doing. How are things going? Are you moving in the right direction? What needs to be changed? We will do this together.

6: WRITTEN REPORT

I will provide you a comprehensive written report of your assessment findings, plan, with recommendations resources, and referrals. Sometimes there's a wild card.

Specialty adult assessments: cognition, living skills, visual-perceptual skills, aging continuum. Pediatric assessments available on a case-by-case basis. In-person ideally. Usually 60 minutes or so. $150

Findings: in-person or virtually. We meet to review and discuss the results from assessment(s) completed. A written report is provided with recommendations. $100

Individual sessions customized to you and your needs. Price varies.

Support pop-ins: price varies.

Small groups on specialty topics and needs: price varies.
